Funding Opportunity: Fiscal Year 2021 National Hypertension Control Initiative Supplemental Funding for Health Centers

The Department of Health and Human Services (HHS) Health Resources and Services Administration (HRSA) and Office of Minority Health (OMH) are partnering through the National Hypertension Control Initiative: Addressing Disparities among Racial and Ethnic Minority Populations (HTN Initiative).1 HRSA plans to offer funding to health centers under this initiative to increase provider and staff engagement in implementing evidence-based practices, including using advanced self-measured blood pressure technology, to increase the number of adult patients with controlled hypertension. Organizations that are a Health Center Program operational (H80) grant award recipient (health center) may request NHCI-HC funds.
