Description

This notice announces the opportunity to apply for funding under the Enhancing Systems of Care for Children with Medical Complexity program. The purpose of this program is to optimize the health, quality of life, and well-being of children with medical complexity (CMC) and their families. The program will accomplish this by funding up to five demonstration projects (HRSA-22-098) to implement and evaluate evidence-informed, patient/family-centered models of care delivery, and by funding a coordinating center (HRSA-22-088) to provide support to the demonstration projects in meeting their program objectives, developing and disseminating resources, and providing leadership in the field Eligible applicants for both HRSA-22-098 (Demonstration Projects) and HRSA-22-088 (Coordinating Center) include any domestic public or private entity, including an Indian tribe or tribal organization (as those terms are defined at 25 U.S.C. § 450b). See 42 CFR § 51a.3(a). Domestic faith-based and community-based organizations are eligible to apply.
